Job summary

We have an exciting opportunity for an ACP to join our Liverpool Community Respiratory Team who provide the Acute Respiratory Infections Virtual Ward.

The Team provides a 2-hour rapid assessment hospital admission avoidance and early supported discharge for patients with exacerbation COPD, bronchiectasis, community acquired pneumonia and viral lower respiratory tract infections, along with nurse led respiratory optimisation in patient's homes and clinic settings. The service is delivered across two hospital sites, clinics and the patients home.

Using Tele medicine, the candidate will have clinical oversight and (in conjunction with Consultants) responsibility for patients who are being managed in their own homes. This will involve working with external agencies and technologies to provide an enhanced Hospital at Home and Early Supported Discharge service for patients with Respiratory conditions

The post holder will provide advanced and expert level of practice (including non-medical prescribing) leading to improved patient outcomes and will act as a professional resource for the multi-disciplinary teampatients and carers, within the scope of their practice and competence,Given the community-wide nature of this role, a high degree of mobility is required. The role will include working across various locations, with shifts covering 7-days per week.

1.To improve the experience and outcomes of care for patients by taking the lead in investigations and treatments prescribed.2. To provide a consistent and personalised service by ensuring that thepatients receive the best care, in the best place from the best person or team.3. To provide social, emotional and psychological support for patients fromreferral and throughout their treatment.4. To deliver expert specialist evidence-based care including development,assessment, planning, implementing, evaluating programmes of evidencebased care for a defined caseload.5. To make appropriate referral to consultants/other specialist/members of themultidisciplinary team in primary, secondary or tertiary care settings.6. To be aware of risk management issues both locally and nationally anddevelop strategies to address identified issues.

To maintain personal accountability for professional and ethical actions andensure compliance with the standards of professional conduct issued by therelevant regulatory body.2. To maintain and develop a specialist service for patients within the caseload,ensuring that the treatment and follow up of this group of patients is deliveredin a timely manner.3. To provide and maintain a diagnostic service for the Trust. Ensure clinicianled clinics provide for both diagnoses and reporting of subsequent results foridentified patients.August 20234. To provide teaching as an in-house measure to all medical and clinical staff,not only within the Care Group but also from other services.5. To provide a clinician-led clinic service to patients.6. To plan and administer treatment to patients as appropriate and ensurehandover of this treatment to community services as necessary.7. To provide expert care, advice and support for patients and theirrelatives/carers and ensure that advice and point of contact is availablethroughout the patients journey.8. To refer on to other agencies as appropriate.9. To operate as clinical specialist and expert practitioner in issues relating tothe identified area of care. To direct and delegate care based on accurateassessment and evaluation.10. To take consent for investigative procedures, manipulate equipment andperform advanced clinical skills.11. To promote effective communication within the hospital and to the patientand/or their relatives.22. Act as a point of liaison between all hospital departments involved in thepatients journey.23. Act as a source of specialist information for clinicians and other members ofthe multidisciplinary team.24. Liaise with other departments/ Trusts regarding services provided and currentpractises to develop best evidence-based practice.25. To be skilled to impart distressing information with respect and empathy.26. To ensure that all patient records are maintained contemporaneously.
