Job summary

Are you a motivated and compassionate Band 6 Nurse looking for a dynamic and rewarding role? The Early Intervention and Prevention Hub (EIPH) offers an exciting opportunity to make a real difference by supporting people to access the right care, at the right time, in the right place.

As an Assessment Nurse, you will play a vital role at the front door of community services, using your clinical expertise and decision-making skills to assess individuals either via telephone or through home visits. You will help shape patient journeys by identifying needs early and directing people to the most appropriate care pathways, helping to prevent unnecessary hospital admissions and promote independence.

Working within a supportive and forward-thinking multi-disciplinary team, you will collaborate closely with colleagues across health and social care, ensuring seamless, person-centred care. You will also have the opportunity to work alongside the Hospital at Home Team, enhancing your assessment and clinical skills in a fast-paced and varied environment.

This post is advertised as Welsh Desirable. Whilst the candidate doesn't need to have skills in Welsh, we'll consider it an advantage when short-listing and selecting candidates. This isn't 'fluency', just Speaking & Listening skills at Level 3 (equivalent to CEFR B2) or above. Level 3 means being able to communicate in Welsh during routine clinical situations, such as welcoming patients, discussing straightforward aspects of their care and answering routine questions. If you're comfortable holding everyday conversations in Welsh, we encourage you to apply. Your Welsh language skills will help us provide person-centred care and enable patients to receive services in their language of choice.
