The Digital Care Coordinator will provide a key digital, care coordination and operational support role across Middleton Integrated Healthcare, working closely with the PCN Manager, Clinical Director, Practice teams, clinical leads, care homes and ARRS teams.

This is a part-time position of 30 hours per week, over 4 days during surgery opening hours.

We are unable to offer visa sponsorship for this role. Applicants must already have the right to work in the UK.

Main duties of the job

The postholder will be responsible for coordinating and supporting the delivery of Middleton Integrated Healthcareservices and projects, ensuring effective use of digital systems, accurate patient and service information, and clear communication between practices, patients, care homes and external organisations.

The role will include responsibility for EMIS and digital support, care home coordination, service and clinic administration, data reporting, Middleton Integrated Healthcare communications and social media, project support/leading, and assisting the PCN management team with operational and HR-related administration.

The postholder will work proactively and independently, managing a varied workload, supporting the delivery of Middleton Integrated Healthcare's priorities and taking the lead on agreed projects and workstreams to ensure they are effectively coordinated and delivered.

Job description

Care Coordination & Care Home Support -Coordinate care home patient lists, coding and alignment with weekly ward rounds; support care home vaccination programmes, including flu and COVID vaccinations, consents and care home planning; liaise with care homes, practices, pharmacy and other relevant services and support the coordination and delivery of Middleton Integrated Healthcare care initiatives.

Digital, EMIS & IT Support -Manage EMIS clinics, user access and associated administrative processes across Middleton Integrated Healthcare; provide Smartcard, IT and digital support to practice and Middleton Integrated Healthcare staff; troubleshoot digital and operational issues; and complete searches, reports and data requirements as needed.

PCN Services, Projects & Performance -Coordinate and support the delivery of PCN services, clinics and projects; lead agreed Middleton Integrated Healthcare projects and work streams; support the PCN with the implementation and delivery of the Network Contract DES; support the PCN Cancer Lead with the delivery of the PCN Cancer Plan, including data, reporting, meetings and actions; support practices to achieve BeCCoR requirements in coordination with the PCN Manager; manage Circulation Health clinics, including bookings, communications, reporting and liaison; and produce clinic utilisation and other performance reports.

Communications, Social Media & Promotion -Take the lead in managing Middleton Integrated Healthcare social media channels, including Facebook and Instagram; create and coordinate content, respond to enquiries and promote PCN services, events and initiatives; produce posters and promotional materials; and support effective communication across Middleton Integrated Healthcare, practices, patients, care homes and external organisations.

Governance & Operational Support -Provide administrative and operational support to the PCN Manager and Clinical Director; support meetings through booking meetings, minute taking, action tracking and follow-up; provide HR-related administrative support, including sickness/absence processes and staff administration; maintain staff calendars and support coordination of team leave and service requirements; and provide day-to-day support to Middleton Integrated Healthcare staff and ARRS teams as required.

Undertake any other reasonable duties within the scope of the role as required by the PCN Manager or clinical Director.

Support changing Middleton Integrated Healthcare priorities, new services, projects and initiatives as required.

Work flexibly across Middleton Integrated Healthcare to provide appropriate digital, administrative, care coordination and operational support.
