About the role

We are looking for an enthusiastic, skilled and motivated Speech & Language Therapistto join our Early Supported Stroke Discharge (ESSD) team, delivering care across Chesterfield and North East Derbyshire.

This is a great opportunity to be part of our well established and highly regarded ESSD model used across Derbyshire. You will play a key role in enabling people to leave hospital earlier and recover safely and effectively in their own homes, delivering high-quality, evidence-based stroke rehabilitation.

Main duties of the job

We are looking for a HCPC and RCSLT registered Speech and Language Therapist, with developed skills, working with communication impairments and dysphagia after stroke, as well as experience working with a community caseload including people with dementia and acquired neurological conditions. You will be able to demonstrate leadership skills in developing service provision and clinical pathways relating to these specialisms. You will have a commitment to providing a high standard of patient focussed care and be a strong team player, with the ability to lead and influence others.

You will be employed by Derbyshire Community Health Services NHS FT as part of their SLT service and as such have access to a team of colleagues with a range of skills and commitment to providing high standards of evidence based approaches, you will be supported by a SLT team leader.

We are a large and dynamic Speech and Language Therapy service of over 100 staff, working across all service areas in Derbyshire.
