Job summary

This is a key role within the pharmacy team delivering services to the Specialist Medicine Directorate. The post-holder will be an active member of both the Pharmacy & Sterile Services Directorate and the HIV / ID and Sexual Health Clinical Service Units (CSU), to ensure continuous two-way exchange of information and maintain high standards of medicines use within the Directorate.

The post-holder will:

1. Participate in, develop and provide clinical pharmacy services to the Infectious Diseases inpatient ward, HIV/ID and Oxford Sexual Health Service clinics

2. Provide pharmaceutical advice on request to GP's, Terence Higgins Trust and other care workers involved in the treatment of HIV.

3. Provide specialised professional and technical education to health care staff and non-NHS staff e.g. THT, Ley community, Oxford Brookes University on drugs in treatment of HIV and HIV related diseases.

4. Provide specialist advice and counselling to patients recieving antiretroviral therapy or TB therapies.

5. Maintain records of patients on antiretrovirals.

6. Promote and ensure the safe and effective use of medicines within the directorate to achieve high standards of medicines related aspects of patient care.

7. Encourage the cost effective use of medicines and to improve the quality of pharmaceutical care for patients. Including management of medication supplies via homecare.

Main duties of the job

Clinical Practice Role

1. To provide a ward-based clinical pharmacy service according to Trust agreed standards including:

• Individual prescription review to optimise therapy
• Confirmation of the patient's medication history
• Advice on dosage, side-effects, cautions and monitoring required
• Advise on administration of medicines
• Appropriate and clear endorsing of prescriptions to ensure safe practice
• Monitor the effect and appropriateness of medication
• Education of patients on their medication
• Involvement in discharge planning and provision of discharge medication

2. To provide clinical pharmacy cover to other ward areas within the team on a reciprocal basis.

3. To ensure medicines are used appropriately, safely and cost-effectively in accordance with Trust policy, standard operating procedures and medicines legislation.

4. To monitor medicines use within the ward area. This includes recording of significant clinical interventions and risk management including:

• Participation in investigating clinical incidents
• Recording significant clinical incidents/near misses
• Ensuring compliance with medicines legislation and local policies

5. To use available information to influence prescribers and ensure the most cost-effective choice of therapy.

6. To participate in ward rounds, clinical meetings or out-patient clinics as appropriate in order to provide pharmaceutical advice to prescribers and other health care professional

Member of the GPhC

Post graduate Diploma in Clinical Pharmacy in Clinical Pharmacy, or equivalent.

MSc in Clinical Pharmacy

Member of the RPS

Experience

Working as a hospital pharmacist

Working as part of a multi-disciplinary team.

Undertaking audit & reporting results

Mentoring or tutoring of pre-registration pharmacists

Clinical experience within the specialist area.

Prescribing in outpatient clinics

Preparation of protocols or guidelines

Presentations or publications at local/regional level

Involvement in Service Development

Preparation of budgetary reports

Accredited Clinical Pharmacy diploma tutor
