Job summary

We are seeking an experienced nurse with a background in diabetes care and a passion for developing others to join our diabetes team in Herefordshire.

This development-focused role works across primary, community and secondary care to strengthen diabetes knowledge, skills and confidence within nursing teams. You will provide mentorship, clinical support and education to Practice Nurses and District Nurses, enabling them to develop their diabetes competencies and deliver safe, high-quality, person-centred care.

You will support Practice Nurse Diabetes Champions across Herefordshire Primary Care Networks, helping them develop their skills and competencies in line with the TREND-UK Integrated Career and Competency Framework for Adult Diabetes Nursing. You will also mentor and support District Nursing teams in diabetes management, including insulin therapy.

Alongside this, you will support patients with complex diabetes needs, using evidence-based practice and patient-centred goals to promote self-management, shared decision-making and improved outcomes, while helping reduce avoidable hospital admissions.

You will have excellent communication skills, the ability to work autonomously and collaboratively across primary, community and secondary care.

Main duties of the job

A key function of this role is to provide clinical support, mentorship, leadership and professional supervision to Practice Nurses supporting insulin and GLP-1 initiation and ongoing management across Primary Care Networks (PCNs). Working collaboratively with Diabetes Specialist Nurses (DSNs), the post holder will support community and district nurses to optimise glycaemic control, improve patient safety and reduce avoidable hospital admissions.

This will include:

o Participating in diabetes MDTs across PCNs, supporting implementation of patient management plans and effective communication with key stakeholders.

o Mentoring Practice Nurse Diabetes Champions and supporting development of skills and competencies in line with the TREND-UK Integrated Career and Competency Framework for Adult Diabetes Nursing, progressing towards Level 2-3 competencies.

o Providing clinical support and mentorship to District Nursing teams to develop confidence and competence in diabetes management, including insulin initiation and optimisation of glycaemic control.

o Promoting person-centred, coordinated care and integration across primary, community and specialist diabetes services, in line with national guidance and best practice.

o Supporting the ongoing development of diabetes transformation across Herefordshire, adapting the role to changing service needs, national guidance and models of care.
