Frailty Health Care AssistantGP Practice

We are currently seeking a Frailty Health Care Assistant (HCA) to join our friendly and supportive team at West Moors Village Surgery. Sadly, our current HCA is retiring to enjoy the good life! We would expect candidates to have experience as an HCA, although not necessarily in a frailty role, as more specific training can be provided. The focus of the role is ensuring the delivery of safe and effective care to our patient population.

 The HCA role involves a variety of duties including Phlebotomy (venepuncture), vaccinations, ECG’s, clinical observations, urinalysis, long-term condition checks and health checks. (This list is not exhaustive).

The fundamental part of the HCA Frailty role is to meet the primary care needs of an identified group of people identified as frail and needing further support in the community. You would be expected to undertake home visits in accordance with the relevant protocols, support patients and families to help patients safely stay at home.

Salary will be dependent on experience and will be discussed at interview. the hours below are subject to negotiation, however, must meet the needs of the practice and service users. However, the total hours of 18hrs per week will not change.

Monday: Day Off

Tuesday: 09:00 – 13:30

Wednesday: 09:00 – 13: 30

Thursday: 09:00 – 13:30

Friday: 09:00 – 13:30
