  • Review and manage patients with long-term conditions and complex healthcare needs.
  • Provide preventative healthcare, health promotion and lifestyle advice.
  • Undertake appropriate clinical examinations and arrange relevant investigations.
  • Review and act appropriately on laboratory results, imaging, correspondence and other clinical information.
  • Provide appropriate follow-up and safety-netting advice to patients.

  • Maintain accurate, contemporaneous and confidential patient records.
  • Ensure appropriate clinical coding and documentation of consultations and clinical decisions.
  • Participate in appropriate clinical and practice meetings.
  • Contribute to clinical audits, significant event reviews and quality improvement initiatives.
  • Contribute to the development and implementation of practice policies and procedures.
  • Participate in clinical governance and patient safety activities.
  • Recognise and appropriately respond to safeguarding concerns.
  • Maintain confidentiality and comply with information governance requirements.
  • Work in accordance with infection prevention and control procedures.
  • Maintain professional registration and undertake appropriate continuing professional development.
  • Keep up to date with relevant clinical guidance, legislation and professional standards.
  • Contribute positively to the practice team and support effective multidisciplinary working.
