Job summary
We are looking for an enthusiastic person who must be able to work in the GP practice for their full hours The Administrative Care Coordinator plays a key role within the Primary Care Network providing high quality administrative coordination and personalised care support to patients and healthcare professionals The role combines general practice administration with care coordination responsibilities supporting the delivery of patient centred services across the PCN
Working closely with GPs Social Prescribing Link Workers Health and Wellbeing Coaches multidisciplinary teams and practice staff the post holder will help patients access the right support at the right time coordinate care and contribute to personalised care planning The role includes a range of operational and administrative duties such as appointment management patient record summarisation processing Subject Access Requests and supporting multidisciplinary team meetings
The Administrative Care Coordinator will help improve patient outcomes reduce health inequalities and support proactive care for people with complex needs long term conditions frailty cancer and care home residents ensuring patients receive coordinated compassionate and effective care across health and community services
Main duties of the job
Main Duties and Responsibilities
- Book appointments for patients requiring review through recall and screening systems.
- Process clinical documents, tasks, referrals and administrative workflows in line with practice procedures.
- Manage incoming and outgoing correspondence, drafting and initiating responses where appropriate.
- Maintain accurate and up to date patient records, ensuring all actions are recorded and completed in a timely manner.
- Summarise patient records and update clinical systems as required.
- Support the processing of Subject Access Requests and other information governance activities.
- Undertake general administrative duties to support the effective day to day operation of the practices and PCN.
- Support seamless pathways between personalised care services and wider health and community organisations.
- Coordinate patient care and ensure actions arising from multidisciplinary team discussions are completed and appropriately documented.
- Assist patients to access appropriate services and support through effective navigation and coordination.
- Contribute to reducing health inequalities by supporting patients with complex needs, long term conditions, frailty, cancer and care home residents.
- Work collaboratively with GPs, practice teams, multidisciplinary colleagues and external partners to ensure patients receive holistic and joined up care.
Job description
The Administrative Care Coordinator will provide a comprehensive administrative and care coordination service across Taunton Deane West Primary Care Network supporting patients healthcare professionals and practice teams to deliver high quality safe and effective care. The post holder will play an important role in ensuring patients receive timely access to services coordinated support and a positive experience throughout their healthcare journey.
The role combines core general practice administration with personalised care coordination responsibilities. The post holder will undertake a wide range of administrative duties including Summarising patients records booking appointments managing recall programmes processing referrals and clinical correspondence maintaining accurate patient records and supporting information governance requirements including Subject Access Requests. They will ensure that all patient information is recorded accurately and that administrative processes are completed efficiently and in line with organisational policies and procedures.
Working closely with GPs practice staff Social Prescribing Link Workers Health and Wellbeing Coaches and wider multidisciplinary teams the Administrative Care Coordinator will support the delivery of personalised care across the PCN. They will assist with coordinating patient care ensuring actions arising from multidisciplinary team meetings case management discussions and personalised care planning are completed documented and communicated appropriately.
The post holder will act as a key point of contact for patients carers and healthcare professionals providing clear information guidance and support. They will help patients understand their care options access appropriate services and navigate health social care and voluntary sector provision. Through effective signposting and coordination they will support individuals to access the services most appropriate to their needs.
A key aspect of the role will involve supporting people to become more confident in managing their own health and wellbeing. This will include helping individuals access self management support educational programmes peer support groups community activities and local services that promote independence wellbeing and resilience. The post holder will contribute to a personalised care approach that places patients at the centre of decisions about their health and care.
The Administrative Care Coordinator will support proactive identification of patients who may benefit from additional support interventions or reviews particularly those living with long term conditions frailty cancer complex health needs or those residing in care homes. They will work collaboratively with clinical and non clinical colleagues to ensure patients receive coordinated holistic and person centred care.
The role requires excellent organisational communication and interpersonal skills together with the ability to manage competing priorities and maintain confidentiality at all times. The post holder will be expected to build effective working relationships across practices and partner organisations and contribute positively to the ongoing development of services across the Primary Care Network.
Through effective administration patient engagement and care coordination the Administrative Care Coordinator will contribute to improving patient outcomes reducing health inequalities enhancing access to services and ensuring the delivery of high quality integrated care for the local population.
