This opportunity is now available in the Chalfont's and Gerrards Cross area to become a Community Nurse within a newly formed team. You will be supported to develop into an autonomous practitioner, nursing patients with multiple complex health care needs in their own home.

Main duties of the job

The post holder will be expected to liaise appropriately with all key members of the multi-disciplinary team and service users related to the speciality.

The post holder is responsible for all aspects of care including the assessment of care needs, planning and implementation / evaluation of care.

The post holder will be expected to carry out all relevant fundamentals forms of care with or without direct supervision maintaining a high standard of nursing care within the framework of Trust policies and in accordance with guidelines as laid down by the NMC.

To support the Charge Nurse/ Sister to monitor the safety and quality of care forpatients.

In the absence of the Deputy Sister/Deputy Charge Nurse to be responsible for the day-to-day management of the ward with support from the Matron as required.

To manage a group of Nursing Assistants and assign delegated tasks and responsibility, as appropriate.

To ensure the compliance of mandatory training and appraisal requirements for delegated Nursing Assistants
