Job summary
Calling all Registered Nurses (Adult, Mental Health or LD) or Allied Health Professionals registered with the HCPC who have an interest in Adults Continuing Healthcare and Funded Nursing Care, including Fast track. The Senior Practitioner role is part of the All Age Continuing Care Team in the cluster ICB of NHS Dorset, NHS Somerset, NHS Bath, NE Somerset, Swindon & Wiltshire. The role operates Monday to Friday core hours, with no night shifts or weekends, with a hybrid approach of office work, remote working and face to face assessments with individuals across the cluster footprint. This role is for a Senior Practitioner, with a requirement for cross cover across the Cluster. Although there will be a primary allocation to a Place team, the expectation is that we will be moving towards one ICB and flexibility is required. The purpose of the role is to undertake Continuing Healthcare (CHC) and Funded Nursing Care (FNC) assessments and reviews on patients who may be eligible or who may already be in receipt of funding this will include those in receipt of fast track funding at end of life and those in receipt of a Personal Health Budget. This will also include the assessment and review of individuals who are in receipt of joint funding (a package of care funded by both health and social care) having previously undergone a CHC assessment whereby health needs were identified that were above the lawful limits of the local authority to provide.
To undertake Continuing Healthcare (CHC) and Funded Nursing Care (FNC) assessments and reviews for individuals who may be eligible or who may already be in receipt of funding this will include those in receipt of fast track and joint funding at end of life and those in receipt of a Personal Health Budget.
To provide and receive highly complex, sensitive and contentious information, communicating unwelcome financial decisions to members of the public and using interpersonal skills to overcome barriers to acceptance where there may be hostility and antagonism.
To case manage individuals in receipt of NHS funded care and support, reviewing and reassessing needs, the service provision and support required, this will include those in receipt of a Personal Health Budget.
To review and reassess individuals eligibility for NHS CHC, FNC and any other NHS funded placements.
To undertake required assessments in support of the Deprivation of Liberty Safeguards (DoLS) ,this may form a key part of the role.
To review any service, and care, commissioned in regards to quality and appropriateness to the individuals need, to ensure that the provider is delivering the requirements of the contract.
To complete retrospective reviews as required and directed by team manager.
To represent the organization at NHE England IRP.
Sign posting to relevant teams and organizations.
Responsible for oversight /assurance of commissioned equipment.
Undertake assessments for retrospective and PUPoC cases.
Contribute to cases that are appealed or disputed.
Commission safe packages of care that meet an individuals assessed care needs in line with delegated financial limit.
Required to maintain constructive relationships with a broad range of internal and external stakeholders.
Participate in relevant internal and external working groups/projects, services, and initiatives which may be highly complex, sensitive, political, and contain contentious information with the aim of providing information and analytical advice to the teams.
Use data collection systems that will provide accurate and timely data e.g. Care Track/IEG4/Other collection tools agreed by leadership colleagues.
Communicate information, risks, issues and dependencies, including briefings and reports to teams and a range of internal and external colleagues.
