Job summary

We are looking to recruit a Lead Practice Nurse to provide clinical leadership to the nursing team and deliver high-quality nursing care to the practice population.

This position is for 24 hours per week.

The post holder will be responsible for managing and developing nursing services, maintaining clinical standards, supporting quality improvement initiatives, and ensuring compliance with regulatory requirements. The post holder must have previous experience working as a Practice Nurse.

The Lead Practice Nurse will work collaboratively with GPs, nurses, pharmacists, healthcare assistants, administrative staff, community teams, and external organisations to provide safe, effective, and patient-centred care.

Main duties of the job

Deliver evidence-based nursing care within scope of practice, including assessment, diagnosis, treatment, screening, health promotion and chronic disease management. Provide care for patients with long-term conditions.

Undertake cervical screening, women's health services, vaccinations and immunisations where appropriately trained. Manage wound care, dressings, minor illness presentations and provide patient education and lifestyle advice.

Act as Immunisation Lead and Infection Prevention & Control Lead. Support safeguarding of children and vulnerable adultsand escalated appropriately.

Maintain and oversee nursing competencies, ensuring staff are trained, competent and appropriately supervised before undertaking delegated duties. Provide clinical supervision, reflective practice and learning opportunities, escalating concerns relating to competence, conduct or patient safety.

Ensure the safe management of medicines, vaccines and emergency equipment, including storage, administration, stock control, cold-chain monitoring, servicing, calibration and record keeping.

Lead infection prevention and control audits, monitor action plans and work with specialist teams to manage risks and outbreaks. Contribute to incident reporting, investigations, significant events, complaints, audits and patient safety improvements.

To maintain current NMC registration and comply with all revalidation requirements throughout employment

Produce accurate, contemporaneous and complete records of patient consultation, consistent with legislation, policies and procedures.

Prioritise, organise and manage own workload in a manner that maintains and promotes quality

Deliver care according to NSF, NICE guidelines and evidence-based care

Ensure accurate notes of all consultations and treatments are recorded adequately on the computer

Participate in home visiting services where required by service need and where appropriately trained and insured

Participate in practice learning and development sessions with other clinical staff

Participate in own personal development and appraisal

Mentor clinical staff where competencies allow

Provide cover for the wider Practice Nursing team as circumstances dictate

CLINICAL RESPONSIBILITIES

Deliver evidence-based nursing care to patients within own scope of practice.

Participate in safeguarding processes for children and vulnerable adults.

Immunisation Lead

Infection prevention and control lead.

Maintain a documented competency framework for nurses, nursing associates, healthcare assistants and trainees, confirming competence before staff undertake new or extended duties.

Delegate duties only to appropriately trained and competent staff, provide suitable supervision and review the outcomes of delegated care.

Provide or arrange regular clinical supervision, reflective practice and protected learning opportunities for the nursing team, escalating concerns about competence, conduct or patient safety appropriately.

Oversee the safe storage, administration, recording and disposal of medicines, including current PGDs and Patient Specific Directions, emergency medicines, anaphylaxis arrangements and the reporting and review of medication or vaccination incidents.

Assist the Practice Manager to co-ordinate national and seasonal immunisation programmes, including vaccine ordering, stock rotation, cold-chain monitoring and audits, management of breaches, vaccine wastage, uptake monitoring and assurance that vaccinators remain trained, competent and authorised.

Maintain an annual infection prevention and control audit programme and action plan, monitor completion of actions, liaise with specialist infection prevention teams and support the management of outbreaks and significant infection risks.

Recognise, document and escalate safeguarding concerns in accordance with practice policies and local pathways, and ensure the nursing team completes required safeguarding training and supervision.

Report and contribute to the investigation of clinical incidents and near misses, implement learning from significant events, complaints, audits and patient safety alerts, and monitor agreed improvement actions.

Ensure clinical and emergency equipment is appropriately checked, serviced, calibrated, cleaned and safe to use, with relevant records maintained and staff trained in its use.

Ensure care is delivered in accordance with requirements relating to informed consent, mental capacity, chaperoning, confidentiality, reasonable adjustments and accessible communication.

LEADERSHIP RESPONSIBILITIES

Provide day-to-day leadership and supervision of the nursing team.

Practice nurse team and HCA Leadership. Lead, support and manage other members of the nursing team, including trainees; to maximise potential within the team.

Ensure skills within the practice nurse team are appropriate and effectively used to deliver care. Support the nursing team in developing their skills and expertise.

Ensure an effective nurse appointment system is in place, in collaboration with the Assistant Practice Manage. Ensure adequate and appropriate nurse cover is available within the practice.

Lead and monitor the development and implementation of policies, standards, and guidance across the practice nurse team.

Lead the practice nurse team in the planning and implementation of local guidance, protocols, and standards, as well as local projects or initiatives.

Carry out annual appraisals and support personal development plans.

Oversee and achieve QOF, IIF and care targets.

Monitor and advise on the effective use of the physical and financial resources by the practice nursing team, including ordering.

Ensure a robust system is in place for the practise nurse appraisals and revalidation.

Enable the practice nurse team to apply and improve clinical governance and risk management systems.

Work to ensure that the Practice remains safe and effective, including the use of clinical audits and learning from significant events and complaints.

Help to ensure the Practice provides a supportive environment for clinicians, trainees, and other employees. Support colleagues' wellbeing, learning and professional development, as well as your own.

Involvement in nursing team recruitment.

Plan nursing capacity and skill mix in response to patient demand, contractual requirements and service priorities, contributing to rotas, leave planning, contingency arrangements and business continuity.

Support recruitment, induction, probation, performance management, succession planning and the development of staff returning to practice or undertaking new competencies.

Advise the Practice Manager and GP Partners on nursing workforce, clinical service and patient safety risks, and contribute relevant matters to the practice risk register.

Ensure appropriate use of the clinical system, templates and coding, monitor the quality of nursing data and recall systems, and support safe digital working and downtime procedures.

Use population-health and clinical data to identify gaps in care and inequalities in access or outcomes, and develop targeted quality-improvement, prevention and health-promotion activity.

Contribute to relevant PCN, ICB, public-health and wider system initiatives, representing the nursing service where agreed.

ADMINISTRATION AND PROFESSIONAL RESPONSIBILITIES

Participate in the administrative and professional responsibilities of the practice team

Ensure accurate and legible notes of all consultations and treatments are recorded in the patients notes

Ensure the clinical computer system is kept up to date with accurate details recorded and amended

Ensure accurate completion of all necessary documentation associated with patient health care and registration with the practice

Ensure collection and maintenance of statistical information required for regular and ad hoc reports and audit

Attend and participate in practice meetings as required
