Job summary
Here at the frailty virtual ward we are looking to recruit a Band 7 Nurse, who has a non-medical prescribing qualification and assessment and consultation course, to come and join our service. The applicant needs to have had at least 3 years of community experience and an extensive understanding of frailty. The post holder will help to drive the team forward, and support by delivering assessments in the patient's normal place of residence. The service works closely with Urgent Community Response Team (UCRT) in helping to prevent unnecessary admissions to hospital and supporting patients to remain at home.
The service is based at Kestrel house in Chelmsford and are co located with UCRT and the district nurses for the area. Multi-disciplinary team meetings are held twice weekly and there are great opportunities for further learning around Frailty, with great support from consultants who work within the team.
Main duties of the job
To undertake comprehensive geriatric assessments for patients under the care of the Frailty Virtual Ward and formulate personalised management plans in collaboration with the MDT. This includes chronic, acute and palliative/end of life care within own competencies, recognising own limitations and seeking advice when necessary. This will include continuously evaluating and acting on outcomes.
Attend as part of a rota, virtual GP calls to discuss patients and liaise with the Frailty Consultant for complex medical management
n Ensure patients receive high standards of clinical care by leading on the development of the skills of the registered staff and support staff, and to participate in training junior colleagues to achieve/maintain their competencies, whilst also leading on discharges alongside the Multidisciplinary team and ensure length of stay is optimal.
Inspiring others through action and example, challenging traditional practices and encouraging innovative problem solving amongst staff
To agree outcomes for people with frailty and complex co-morbidities and reduce the need for these people to attend or be admitted into hospital or long-term residential care
Develop innovative ways of increasing flow in and out of the frailty virtual ward with a particular focus on admission avoidance working in collaboration with the Urgent Community Response Team (UCRT)
