Job summary

An exciting opportunity has arisen for a motivated, compassionate and patient focused GP to join our dynamic team for four to six sessions per week.

We are looking for a GP who shares our commitment to providing high quality, holistic and patient centred care and who will enjoy working as part of our supportive multi disciplinary team.

The successful candidate will provide the full range of general practice care, including acute consultations, management of long term conditions, review and actioning of results, occasional home visits and participation in our planned care home ward rounds, providing proactive and coordinated clinical care to residents.

The successful candidate will be joining an established multi disciplinary team and will have opportunities to contribute to clinical governance, quality improvement, shared learning and the ongoing development of our services.

Main duties of the job

The successful candidate will provide high quality, safe and effective clinical care to our patients, including consultations, examinations, diagnosis, treatment, follow up and appropriate referral.

They will manage acute presentations and long term conditions, developing personalised care plans and supporting patients to make informed decisions about their health.

The role will include occasional home visits, alongside participation in a regular care home ward round, working collaboratively with the wider multidisciplinary team to provide proactive, person centred care to residents. This will include clinical review, management of acute and long term conditions, medication review where appropriate, advance care planning and coordination with care home staff and other health and care professionals.

The postholder will review and action investigations, correspondence and results in a timely manner, undertake appropriate prescribing and medicines management, and maintain accurate and contemporaneous clinical records.

As a member of the clinical team, the postholder will participate in practice meetings, clinical governance, audit, significant- event learning and quality improvement activity.

They will maintain appropriate continuing professional development and participate fully in appraisal, revalidation and other professional requirements.

Job description

The following are the core responsibilities of the Salaried GP. The postholder, may, on occasion, be required to undertake other reasonable duties appropriate to the role, depending upon the needs of the practice.

The postholder will:

a. Provide high quality, safe and effective medical care to the practice population, including assessment, diagnosis, treatment, follow up and appropriate referral.

b. Provide services in accordance with the practice's GMS contract and relevant local and national requirements.

c. Manage acute presentations and long term conditions, supporting patients to make informed decisions about their care and developing personalised care plans where appropriate.

d. Prescribe safely and effectively in accordance with current clinical guidance, local prescribing arrangements and practice policies.

e. Review and action investigations, test results, clinical correspondence, referrals and other associated clinical administration in a timely manner.

f. Maintain accurate, clear and contemporaneous clinical records in accordance with professional standards, legislation, practice policy and guidance.

g. Undertake occasional home visits, normally 0-1 per day, according to clinical need.

h. Participate in a regular care home ward round, providing proactive and coordinated clinical care to residents. This will include clinical review, management of acute and long term conditions, medication review where appropriate, advance care planning and liaison with care home staff and other members of the multidisciplinary team.

i. Participate where required, in the practice's arrangements for urgent and same day care, including telephone consultations and duty doctor responsibilities.

j. Work collaboratively with GP colleagues, nursing and wider multidisciplinary teams to provide coordinated, person centred care.

k. Contribute to the delivery of QOF, enhanced services and other relevant contractual or quality requirements, including taking responsibility for agreed areas where appropriate.

l. Participate in clinical audit, quality improvement and other initiatives designed to improve patient care and practice services.

m. Participate in significant event reviews, learning events and other clinical governance activities, contributing openly and constructively to shared learning.

n. Attend and contribute to practice and clinical meetings as required.

o. Contribute to the development and maintenance of high standards of clinical governance, patient safety and quality across the practice.

p. Work in accordance with practice policies and procedures and relevant professional, regulatory and statutory requirements.

q. Maintain professional registration and comply with all requirements relating to appraisal, revalidation and continuing professional development.

r. Contribute where appropriate, to the education and development of colleagues, students and other healthcare professionals.

s. Work collaboratively with colleagues across the practice and wider primary care team, contributing to continuous improvement and the development of services.

Secondary responsibilities.

In addition to the primary responsibilities, the Salaried GP may be requested to:

a. Participate in practice audits and quality improvement projects.

b. Participate in local and PCN initiatives designed to enhance service delivery and patient care.

c. Contribute to the review of significant events, patient safety incidents and near misses using the practices agreed learning and review processes.

d. Contribute to the development and review of clinical protocols, pathways and practice policies.

e. Support agreed practice and PCN priorities and service developments appropriate to the role.

Please note that Huntcliff Surgery is not currently able to offer Skilled Worker visa sponsorship for this post. Applicants must therefore already have the right to work in the UK without requiring sponsorship from the practice.

We also benefit from a strong wider workforce, including ARRS colleagues such as First Contact MSK Practitioner, Mental Health Practitioners and Social Prescriber, alongside our management and administrative teams.
