The post holder will support the wider community paediatric team in providing nurse led clinics to support children with complex, long term health conditions, with guidance from the paediatricians, with particular attention to the physical health aspect if the young person.
Main duties of the job
To support the Community Paediatrics Team in the provision of a high-quality service to children, young people and their families, who require health assessments, investigation and treatment for children and young people with long term health and neurodisability condition's.
To provide nurse led clinics for children with identified long terms conditions.
Provide advice and support to junior staff and learners in all aspects of the delivery of high-quality care.
Provide support, advice and education to children and young people and their families in the management of their care.
Work closely in partnership with other multi-disciplinary team members to ensure effective care planning and delivery.
Service hours are Monday to Friday, 9am - 5pm.
Job description
- Support and manage onward referrals to other professionals as required to support child and young person care.
- Using evidence-based practice to support the management of care
- Provide clinical information to support education and health care plans (EHCPs) as requested.
- Screen and/or redirect referrals for children and young people over 5 years of age.
- Work in accordance with the NMC code of professional practice and scope of professional practice. Professional and legally accountable and responsible for all activities.
- Provide clinical expertise and knowledge, demonstrating a sound understanding of the issues related to the identification, assessment, treatment and management of the client group and demonstrate the ability to work autonomously and manage own caseload.
- Act as a role model at all times and ensures that all team members and others visiting the area give a high priority to policies and procedures relating to infection control in order to maintain a safe clinical environment for everyone.
