We are looking for an enthusiastic and motivated Lead Pharmacist to join our team and provide clinical leadership across our anticoagulation services.
This is an exciting opportunity for a pharmacist who is passionate about improving patient outcomes, optimising medicines use and developing high-quality services.
You will play a key role in leading and developing the service, ensuring the delivery of safe, evidence-based and person-centred care to patients requiring anticoagulation.
You will work collaboratively with multidisciplinary teams, healthcare professionals and patients to optimise anticoagulant therapy, promote best practice and drive continuous service improvement.
Main duties of the job
Provide clinical and professional leadership for anticoagulation services.
Support the safe and effective use of anticoagulants.
Lead the development, implementation and evaluation of anticoagulation pathways and service improvements.
Provide expert advice and education to healthcare professionals, patients and carers.
Promote medicines optimisation, patient safety and adherence to relevant guidelines and policies.
Work collaboratively with medical, nursing, pharmacy and wider multidisciplinary teams.
Support audit, quality improvement and clinical governance initiatives.
Contribute to the education, training and development of pharmacy colleagues and other healthcare professionals.
Job description
CLINICAL
Provide, lead and develop the clinical pharmacy service to the specialist area in line with patient and departmental needs, to ensure the delivery of a high quality, patient centred and effective clinical pharmacy service.
Contribute effectively to patient care as part of the multidisciplinary team, to make proactive interventions in individual patients therapy and to provide information on drug related issues and to act as the lead pharmacy contact for the specialist area.
Contribute to the multidisciplinary identification, review and development and ratification of treatment guidelines and policies in the specialist area, in order to promote safe and evidence-based patient care.
LEADERSHIP AND MANAGEMENT
Lead and manage the work of all the pharmacists, clinical pharmacy technicians and medicines optimisation assistants working in the specialist area, to maintain and further develop a high-quality pharmacy workforce.
Participate in and deliver to line managed staff, the departmental individual performance review programme to ensure that staff performance and development is maintained as a high priority in line with the needs of the service.
Participate in the recruitment, selection and retention of clinical pharmacists and pharmacy technicians, in order to maintain and further develop a high quality pharmacy workforce.
EDUCATION & TRAINING
Manage, within the specialised area, the teaching of Trainee Pharmacists and Early Careers Pharmacists, Pharmacy Technicians and Pharmacy Assistants to support them in their induction, development and obtaining their accreditations.
Ensure pharmacy staff are updated on clinical guidelines used in the specialist area, through the participation in training programmes and through the provision of specialist medication information and standard operating procedures.
Provide multidisciplinary education and training on all aspects of medicines management.
RESEARCH AND DEVELOPMENT
Contribute to research within the pharmacy department. This includes identifying areas of research, including identifying gaps in evidence, applying appropriate methods to address identified gaps and generating new evidence.
Share good practice by publications and presentations within the Trust, throughout the local health economy and nationally.
PROFESSIONAL

Recognise and respect the role of others in the wider pharmacy and multi-disciplinary team.
Participate in late night, weekend, Bank Holiday and on call rota duties.
