Job summary
An exciting opportunity has arisen for an Integrated Neighbourhood Team Lead (x2) to support the development and delivery of Neighbourhood-based health and care across Great Yarmouth and Waveney.
Norfolk and Waveney are adopting a neighbourhood health approach, bringing services closer to communities and enabling more joined-up, preventative and person-centred support. Integrated Neighbourhood Teams will play a key role in making this ambition real by bringing together partners from primary care, community services, social care, mental health, local authorities, voluntary and community organisations, and wider system partners.
The postholder will provide senior leadership within neighbourhoods, supporting partners to work as one system around local people and communities. This is a key role for someone who can operate across organisational boundaries, build trusted relationships, use data and community insight to drive improvement, and translate strategic priorities into practical delivery.
Please note - the base has yet to be confirmed (due to 2 available positions).
The post holder will be required to work across geographical locations.
Interviews are currently expected to take place the week commencing the 2nd November 2026.
Main duties of the job
The Integrated Neighbourhood Team Lead will provide strategic and operational oversight for the delivery of the Integrated Neighbourhood Team model within a defined neighbourhood.
The postholder will work with partners to support effective MDT working, strengthen pathways and escalation routes, and ensure that neighbourhood health delivery is aligned to local need, population health priorities and wider system objectives.
Key responsibilities will include:
- Providing leadership and oversight for Integrated Neighbourhood Team delivery, supporting collective accountability for performance, quality, consistency and outcomes.
- Developing and assuring effective multidisciplinary team ways of working, including clear roles, decision-making arrangements, information flows and escalation routes.
- Acting as a trusted system facilitator, supporting collaboration across NHS, local authority, social care, mental health, VCSE, primary care and community partners.
- Ensuring neighbourhood delivery is shaped by community insight, lived experience and co-designed approaches with residents and partners.
- Using population health data, performance information, resident feedback and partner insight to identify risks, opportunities and areas for improvement.
- Pathway and process improvement so that care and support is more coordinated, proactive and person-centred.
- Providing a supportive an collaborative approach to staff within the Integrated Neighbourhood Team.
Job description
Operational Coordination
- Provide strategic and operational oversight for INT delivery, working with partners to ensure collective accountability for performance, consistency, and outcomes across Multidisciplinary Teams (MDT)
- Define, implement and assure standards for multidisciplinary team effectiveness, intervening by exception where performance, quality or consistency fall below expectation
- Ensure referral pathways, escalation routes and handovers are clear, consistently applied and understood by all partners
- Monitor operational pressures (capacity, demand, bottlenecks) and take early action to resolve issues
- Provides strategic oversight of operational challenges, intervening by exception where system barriers persist
Integrated System & Effective MDT Working
- Hold and communicate shared priorities across all partner organisations, ensuring alignment with national and local requirements.
- Oversees the development and consistent application of MDT ways of working, including clarity of roles, responsibilities, decision making authority and escalation routes across partner organisations.
- Provides assurance that MDT processes, tools and information flows are fit for purpose, addressing variation and promoting consistency across neighbourhoods.
- Acts as a system facilitator, enabling constructive collaboration across organisational cultures, priorities and professional perspectives to support integrated neighbourhood delivery, integration & removing operational silos
System & Partnership Facilitation
- Lead coordination not only of INT delivery but its alignment with Local Area Coordination, community-led approaches, and wider neighbourhood infrastructure
- Ensure neighbourhood delivery is shaped by community insight, lived experience and co-designed approaches with residents and VCSE partners
- Influence cross-sector partners to align priorities, resolve competing demands and co-own neighbourhood delivery, acting as a trusted system leader rather than a directive authority
Build and maintain effective working relationships across:
Primary care / Primary Care Networks (PCNs)
Community services
Social care
Mental health services
VCSE organisations
District and place-based partners
- Facilitate collaboration between organisations with different incentives, pressures and constraints
- Act as a trusted operational broker, supporting shared problem solving rather than enforcing solutions
- Represent the INT in relevant operational and neighbourhood forums
- Enable Effective Case Management
- Provide strategic oversight and assurance of neighbourhood case management arrangements, ensuring systems and processes support effective coordination for complex cohorts
- Sets clear expectations for how INT Coordinator & Case Managers interact with MDTs and partner organisations, promoting consistency, accountability and timely multi agency responses.
- Uses insight from the INT Coordinator / Case Manager activity to identify recurring system barriers, informing pathway improvement, partner discussions and escalation where required.
- Assures that the INT Coordinator & Case Managers are supported by effective pathways, partner engagement and system processes, intervening at a system level where issues cannot be resolved locally.
Data, Insight & Performance Management
- Leads the use of population health data, and insight to assess performance, identify emerging risks and inform strategic decision making.
- Create and facilitate reflective learning environments where partners review delivery, share insight and co-develop solutions to neighbourhood challenges
- Incorporate qualitative insight from residents, communities and partners alongside data to inform evaluation and continuous improvement
- Provides assurance on neighbourhood outcomes and delivery effectiveness, translating insight into clear actions, recommendations and escalation where required.
- Promotes a culture of evaluation and continuous learning, ensuring learning from delivery is captured, shared and used to refine pathways, workforce approaches and partnership arrangements.
- Analyse data including complaints and patient/carer feedback to drive improvement.
- Produce high quality reports, briefings and performance summaries for senior leaders and system partners.
- Pathway & Process Management
- Provides strategic oversight of neighbourhood pathways, leading system-wide redesign to ensure alignment with population need, neighbourhood priorities and system objectives, with ongoing review to support continuous refinement
Workforce Leadership & Culture
- Provide day to day leadership for staff within the INT, creating a supportive, collaborative and high performing culture.
- Ensure staff are equipped with the skills, tools and information required to deliver safe and effective discharge processes.
- Support workforce planning, development and wellbeing, including preparation for seasonal demand and surge periods.
- Promote cross organisational working, shared learning and consistent standards across all teams within the integrated hub.
Governance, Risk & Assurance
- Identify risks and issues early, ensuring mitigating actions are agreed, monitored and escalated where required.
- Ensure compliance with local policies and system wide governance frameworks.
- Contribute to system wide planning, commissioning and performance discussions, providing local intelligence and operational insight.
- Support partners to deliver safe, person centred processes that improve patient, family and carer experience.
- Ensure learning from operational issues is shared
- Ensure information sharing and data use support integrated working and are compliant with requirements
- Maintain clear operational records, action logs and decision trails where required
- Support alignment between INT delivery and wider place / system priorities
