Salary: £44,910 - £46,385 per annum (Inclusive of 7.5% market supplement)
Hours per week: 37 hours

Our Continuing Healthcare Team is looking for a Level 3 Social Worker. The Continuing Healthcare (CHC) team works across the county to assess and review adults who may be eligible for NHS Continuing Healthcare funding. The team works closely with health and social care partners, providing specialist advice on eligibility, funding arrangements, and care planning to ensure individuals receive the appropriate support and funding.
Main duties of the job
The Continuing Healthcare Social Worker will play a key role in assessing individuals with complex health and care needs, supporting CHC eligibility decisions, and ensuring appropriate funding arrangements are in place. Working closely with health and social care professionals, they will undertake person-centred assessment, provide specialist CHC advice, manage risks and safeguarding concerns and help people access the support they need while promoting independence and positive outcomes.
The successful candidate will be a qualified and registered Social Worker with Social Work England, possessing up-to-date knowledge of safeguarding, health and social care legislation and relevant policy. Experienced in supporting vulnerable adults, committed to ongoing professional development and skilled in communication.
This position comes with a 7.5% market supplement payment. All market supplement payments are subject to review on an annual basis.
