South Doc Services (SDS) is hiring an Advanced Medical Practitioner (AMP) on behalf of West Heath Surgery.

This will be a full time, permanent role with a 37.5 hours per week contract arrangement. The job location is at West Heath Surgery, 194 - 196 West Heath Road, Birmingham, B31 3HB. Pay / Salary is dependent on experience level.

The Advanced Medical Practitioner (AMP) will provide high-quality, safe, effective and person-centred clinical care to patients registered with West Heath Surgery who reside in designated care homes.

The post-holder will work as an integral member of the Practice multidisciplinary team, working collaboratively with GPs, advanced practitioners, practice nurses, clinical pharmacists, community nurses, therapists, care coordinators, social care professionals, care-home staff and other relevant health and social care professionals.

Main duties of the job

The post-holder will contribute to the delivery of proactive, personalised and coordinated healthcare for care-home residents, with an emphasis on:

• prevention of avoidable deterioration;
• early identification and management of acute illness;
• proactive management of frailty and long-term conditions;
• personalised care and support planning;
• medicines optimisation in collaboration with the appropriate prescriber/pharmacy professional;
• reducing avoidable hospital admissions;
• facilitating timely access to secondary and community care;
• supporting residents to remain in their preferred place of care wherever clinically appropriate;
• effective multidisciplinary working;
• continuity and coordination of care; and
• safe, evidence-based clinical decision-making.

The role will support the Practice in delivering its contractual requirements under the current NHS England Network Contract DES and Enhanced Health in Care Homes arrangements. The current EHCH model is designed around proactive, personalised and integrated care for people living in care homes rather than a predominantly reactive model.

The post-holder will work within their individual scope of professional practice, competence and authority. Where an activity requires GP, consultant, specialist or other professional input, the AMP will recognise this promptly and escalate appropriately.

Job description

Principle Duties and Responsibilities:

The post-holder will support delivery of the Practice EHCH service in accordance with:

• the current NHS England Network Contract DES;
• the current Enhanced Health in Care Homes framework;
• the Practices agreed care-home operating model;
• the Practice's clinical governance arrangements;
• relevant ICB requirements and local pathways;
• NICE guidance and other national clinical guidance;
• relevant safeguarding, Mental Capacity Act and deprivation of liberty requirements;
• information governance requirements;
• local medicines-management policies;
• relevant CQC requirements and care-home policies; and
• professional standards and regulatory requirements.

The AMP will:

• Participate in scheduled care-home ward rounds as part of the multidisciplinary team and according to the agreed Practice care-home model.
• Work collaboratively with care-home staff to identify residents requiring clinical review, prioritising according to clinical urgency, frailty, risk and agreed care-home processes.
• Contribute to proactive identification of residents at increased risk of deterioration, hospital admission, falls, medication-related harm, malnutrition/dehydration, pressure damage and other significant health risks.
• Contribute to personalised care and support planning, ensuring that the resident's wishes, preferences, capacity, communication needs, clinical needs and goals are considered.
• Support timely clinical assessment of new or returning care-home residents and contribute to the Practice's arrangements for comprehensive assessment and personalised care planning following admission/readmission.
• Review and update care plans where there is a significant change in clinical circumstances.
• Work with the wider MDT to identify appropriate interventions and services that enable residents to remain safely within the care-home environment wherever appropriate.
• Participate in MDT discussions and case conferences for residents with complex or changing needs.
• Support effective communication between the care home, Practice, community services, secondary care, pharmacy, social care and other relevant agencies.
• Contribute to service evaluation, audit, quality improvement and monitoring of EHCH activity and outcomes.

The current national framework emphasises integrated working between primary care, community health, social care and care homes, with personalised care and support at the centre of the model.
