We are looking for an enthusiastic and motivated Salaried GP to join our friendly, supportive and forward-thinking team.
Dr H Nay Lin & Partner is a well-established GMS practice in West Ealing, serving a diverse patient population. We pride ourselves on high-quality, patient-centred care and a positive, collaborative working environment.
Our experienced multidisciplinary team includes a Practice Nurse, Healthcare Assistants, Clinical Pharmacist, ARRS GP, Mental Health Care Coordinator and a strong practice management and administrative team. GPs benefit from excellent support with workflow, including incoming correspondence, clinical coding, referrals, pathology results and fit notes. Our in-house pharmacy team supports repeat prescriptions and medication queries, helping GPs focus on clinical care.

Main duties of the job
The Salaried GP Role is in line with the BMA Salaried GP Model Contract.
The roles and responsibilities are in keeping with the expectations of a salaried GP including but not limited to face to face and remote assessment, document, blood, and ECG reviews, prescriptions reviews, home visits (minimal as we have a PCN home visiting team), and QOF, Ealing Standards, MOES and CRM contribution.

Job description
Core ResponsibilitiesThe Salaried GP will be expected to undertake a broad range of clinical, administrative and professional responsibilities in support of the delivery of safe, effective and high-quality general practice. The duties outlined below represent the core expectations of the role. From time to time, additional responsibilities may be required in response to changing patient needs, workload, staffing arrangements, service developments or other operational requirements.
Clinical Care
- Provide comprehensive, evidence-based and patient-centred general medical services to the practice population, managing patients presenting with a wide range of acute, chronic and complex conditions.
- Undertake appropriate clinical assessment, diagnosis, treatment and follow-up, making effective use of available clinical and community services.
- Recognise and appropriately manage patients whose clinical presentation requires urgent assessment, specialist input or onward referral.
- Ensure that clinical decisions are made in accordance with professional standards, relevant legislation, national guidance and locally agreed pathways.
- Support appropriate prevention and health promotion activity, including screening, vaccination, lifestyle advice and identification of patients at increased clinical risk.
- Provide continuity of care wherever reasonably practicable, particularly for patients with complex or ongoing healthcare needs.
- Work within the limits of professional competence and seek appropriate advice or support when required.
Prescribing and Medicines Management
- Prescribe safely, responsibly and cost-effectively, taking account of individual patient circumstances and current clinical evidence.
- Follow relevant national and local prescribing guidance, formularies, shared-care arrangements and practice policies.
- Review medication appropriately, particularly where patients have complex or multiple medication requirements.
- Identify and act on potential prescribing risks, interactions, contraindications or medication-related concerns.
- Contribute to initiatives designed to improve medicines optimisation, prescribing quality and patient safety.
Management of Long-Term Conditions
- Provide appropriate ongoing management and review of patients with long-term conditions in accordance with evidence-based clinical guidance.
- Contribute to the achievement of relevant clinical indicators and quality standards, including QOF and other locally agreed programmes.
- Support proactive identification and recall of patients requiring monitoring or review.
- Work collaboratively with nurses, pharmacists, healthcare assistants and other members of the multidisciplinary team to ensure coordinated care.
- Encourage patients to participate actively in decisions about their health and ongoing management.
Clinical Records and Information Management
- Maintain accurate, complete and contemporaneous clinical records for every patient contact.
- Ensure that clinical documentation clearly reflects assessment, decision-making, treatment, follow-up arrangements and safety-netting where appropriate.
- Ensure that referrals, investigations, correspondence and other clinical information are appropriately recorded and acted upon.
- Maintain confidentiality and comply with information governance requirements at all times.
- Use the practice's clinical systems effectively and in accordance with agreed procedures.
Referrals, Investigations and Clinical Administration
- Complete referrals, clinical correspondence, investigation requests and other patient-related administration accurately and within appropriate timescales.
- Review and act upon investigation results, hospital correspondence and other incoming clinical information in accordance with practice procedures.
- Ensure appropriate follow-up arrangements are communicated clearly to patients and relevant members of the practice team.
- Respond appropriately to prescription requests and other clinical administrative tasks within agreed practice processes.
- Recognise that clinical administration forms an integral part of safe patient care and should be managed promptly and effectively.
Team Working and Practice Contribution
- Work collaboratively with GP partners, salaried GPs, nursing staff, pharmacists, healthcare assistants, administrative colleagues and the wider multidisciplinary team.
- Participate fairly in the distribution of the practice's clinical workload and contribute to the overall functioning of the service.
- Communicate effectively with colleagues and contribute to a positive, respectful and supportive working environment.
- Attend relevant practice meetings and contribute constructively to discussions concerning patient care, service delivery, governance and development.
- Support colleagues where appropriate while maintaining clear professional boundaries and individual accountability.
- Contribute to effective multidisciplinary working both within the practice and with external healthcare and community services.
Clinical Governance, Quality and Audit
- Practise in accordance with current evidence, professional standards, national guidance and locally agreed clinical pathways.
- Participate in clinical audit, significant event review, learning from incidents and other clinical governance activities as required.
- Contribute to the identification of risks and opportunities for improving patient safety and quality of care.
- Support the implementation and monitoring of agreed quality improvement initiatives.
- Participate in practice reviews of clinical performance and outcomes where appropriate.
- Take responsibility for agreed areas of clinical governance, quality improvement or service development where delegated.
- Contribute to the delivery and monitoring of relevant QOF requirements and other contractual or quality standards.
- Identify opportunities to improve clinical pathways, patient experience, access and outcomes.
Quality Improvement and Service Development
- Engage positively with changes designed to improve the quality, efficiency and accessibility of practice services.
- Contribute ideas and feedback to support the development of new clinical pathways, digital systems and models of care.
- Participate in agreed improvement projects and evaluate the impact of changes where appropriate.
- Support the practice in responding to changes in national NHS policy, contractual requirements and local commissioning arrangements.
- Demonstrate a willingness to adapt working practices where this improves patient care, safety or service effectiveness.
Professional Development, Appraisal and Revalidation
- Maintain professional registration and comply with all relevant requirements of the General Medical Council and other applicable regulatory bodies.
- Participate fully in the practice appraisal process and maintain an appropriate personal development plan.
- Undertake continuing professional development relevant to the requirements of the role and identified learning needs.
- Maintain appropriate evidence of learning, reflection and professional development to support appraisal and revalidation.
- Complete all mandatory training and other training identified as necessary by the practice.
- Remain up to date with developments in clinical practice relevant to the GP role.
- Demonstrate reflective practice and apply learning from clinical experience, audit, incidents, complaints and feedback.
- Take responsibility for maintaining appropriate levels of clinical competence and seek additional training or supervision where required.
Policies, Procedures and Professional Standards
- Familiarise themselves with and comply with the practice's policies, protocols, standard operating procedures and clinical guidance.
- Work within applicable NHS requirements, professional guidance, statutory obligations and practice governance arrangements.
- Raise concerns appropriately where there are issues relating to patient safety, clinical practice, staffing, safeguarding or service delivery.
- Maintain professional behaviour and uphold the principles of equality, dignity, respect and confidentiality in all interactions.
- Ensure that personal conduct contributes positively to the reputation and effective functioning of the practice.
Safeguarding and Patient Safety
- Maintain awareness of safeguarding responsibilities and act appropriately when concerns relating to children, adults at risk or other vulnerable patients arise.
- Follow the practice's safeguarding policies and escalation procedures.
- Recognise and respond appropriately to clinical risk, including deterioration, diagnostic uncertainty and concerns regarding patient safety.
- Ensure appropriate safety-netting advice is provided and documented where clinically indicated.
- Report and participate in the investigation and learning from significant events, patient safety incidents and complaints as required.
Communication and Patient Experience
- Communicate clearly, respectfully and compassionately with patients and their relatives or carers.
- Involve patients in decisions about their care and take account of their individual circumstances, preferences and needs.
- Manage consultations effectively while maintaining appropriate clinical standards and patient safety.
- Promote a culture in which patients feel listened to, respected and involved in their care.
- Respond professionally to concerns, complaints and feedback and participate in learning arising from these where appropriate.
