Job summary

As a Practice Nurse at Springfield Surgery, you will play an important role in delivering high-quality nursing care and supporting the health and wellbeing of our patients. Working closely with GPs, nurses and other healthcare professionals, you will help provide safe, effective and compassionate care to our local community.

This is a part-time position of up to 20 hours per week, usually worked over two days. We can offer some flexibility in the working pattern for the right candidate.

Main duties of the job

As a Practice Nurse, you will provide a broad range of nursing care within a busy GP practice, including chronic disease reviews, cervical screening, childhood immunisations, travel health, vaccinations, wound care and health promotion.

You will work closely with GPs, nurses, healthcare assistants and the wider practice team to support patients with both routine and ongoing healthcare needs. The role requires confident clinical decision-making, good communication skills and the ability to recognise when further medical assessment is needed.

Job Purpose

To provide safe, effective and caring nursing services to patients registered at the practice. The Practice Nurse will work as part of the wider practice team and support the delivery of high-quality patient care.

Main Duties

  • Provide a range of Practice Nurse clinics and appointments, assessing patients and delivering care within agreed protocols and professional competence.
  • Carry out cervical screening, including taking cervical smear samples, providing appropriate advice and ensuring correct follow-up procedures are followed.
  • Deliver childhood immunisation programmes in accordance with the current UK vaccination schedule and practice procedures.
  • Provide routine adult vaccinations, including seasonal vaccination programmes and other vaccinations recommended under national guidance.
  • Provide travel health advice, risk assessments and travel vaccinations in line with current guidance and practice protocols.
  • Support the management and monitoring of patients with long-term conditions, including conditions such as asthma, COPD, diabetes and cardiovascular disease, in accordance with agreed care plans and the nurse's level of training and competence.
  • Carry out health checks, reviews and basic nursing assessments, including blood pressure, weight, BMI and other clinical observations.
  • Take blood samples and carry out other clinical investigations as required.
  • Provide wound assessment and management, including dressings and the removal of sutures, staples or clips.
  • Administer prescribed injections and medicines in accordance with Patient Specific Directions, Patient Group Directions and practice procedures, where appropriate.
  • Provide health promotion and lifestyle advice, including smoking cessation, healthy eating, exercise and preventative healthcare.
  • Recognise changes or concerns in a patient's condition and refer or escalate to a GP or other appropriate clinician when required.
  • Maintain accurate, timely and clear clinical records using the practice clinical system.
  • Follow infection prevention and control procedures and maintain high standards of cleanliness and clinical safety.
  • Ensure clinical rooms, equipment, vaccines, medicines and stock are stored, checked and maintained appropriately, including cold-chain requirements.
  • Participate in recalls, reviews and follow-up of patients where appropriate, including cervical screening, immunisations and long-term condition monitoring.
  • Work closely with GPs, other Practice Nurses, Healthcare Assistants, reception staff and the wider primary care team to provide coordinated patient care.
  • Participate in clinical meetings, audits, quality improvement work and practice initiatives as required.
  • Maintain professional competence through mandatory training, continuing professional development and appropriate clinical updates.
  • Follow practice policies and procedures relating to confidentiality, consent, safeguarding, information governance, health and safety and professional conduct.

The postholder must work within the NMC Code, practice policies and relevant legal and professional requirements. Duties may be updated following discussion to meet the changing needs of the practice.
