Job summary

Whitstable Medical Practice is looking for a motivated, compassionate and clinically confident Practice Nurse with experience in Chronic Disease Management to join our friendly and supportive multidisciplinary team.

This is an exciting opportunity for a nurse who enjoys developing long-term relationships with patients and has a particular interest in the management of Asthma, COPD and/or Diabetes, alongside the wider range of services delivered within primary care.

We are dedicated to delivering high quality care to our 45,500 patients across three sites: Estuary View Medical Practice, Chestfield Medical Centre and Whitstable Medical Centre.

The successful candidate will be required to work flexibly across all three locations, as required. We welcome applicants from candidates seeking full time hours, we would also consider two part time applicants.

The successful candidate will be supporting the Practice's chronic disease management programme, helping patients to manage their conditions effectively, improve their health and wellbeing, and reduce the risk of complications.

Job description

Main Duties of the Job

The successful candidate will undertake autonomous clinical practice within their professional scope and will contribute to the delivery and development of chronic disease services across the Practice.

Key responsibilities will include:

Taking a lead role in the management and review of patients with Asthma, COPD and/or Diabetes, in line with current evidence-based guidance.

• Undertaking comprehensive clinical assessments and developing individualised management and treatment plans.
• Monitoring disease control, treatment effectiveness and patient outcomes.
• Supporting patients to understand and manage their long-term conditions, encouraging self-management and healthier lifestyle choices.
• Identifying patients at increased risk of complications or deterioration and taking appropriate action.
• Recognising when referral to GPs, specialist teams or secondary care is required and ensuring appropriate onward referral.
• Supporting the identification and proactive management of patients at risk of developing long-term conditions.
• Providing health promotion and preventative healthcare advice.
• Working collaboratively with GPs, Advanced Practitioners, Pharmacists, Healthcare Assistants and the wider Practice team.
• Requesting and reviewing pathology investigations where appropriate and acting on results within scope of practice.
• Maintaining accurate, contemporaneous and confidential clinical records in accordance with professional and legislative requirements.
• Supporting patients with complex and long-term health needs and liaising with community and specialist services where appropriate.
• Contributing to safeguarding processes and supporting the wider clinical team with safeguarding concerns.
• Delegating appropriate clinical tasks to members of the nursing and healthcare support team.
• Participating in clinical audit, quality improvement and the development of Practice clinical services.
• Maintaining professional knowledge and competence through continuing professional development.

Contributing to the wider Practice Nursing service, according to skills and experience, including:

• Cervical screening
• Immunisations and vaccinations
• ECGs
• Wound care and dressings
• Health checks
• Routine nursing clinics
• Other appropriate Practice Nurse services

Job Responsibilities

The successful candidate will be a professional, motivated and patient-focused Registered Nurse who is able to work confidently and autonomously while also contributing positively to our multidisciplinary team.

You will be expected to deliver safe, effective and compassionate nursing care to our registered patient population and to work within your professional scope of practice.

Depending on your skills, experience and areas of clinical interest, there will be opportunities to develop your role and take responsibility for specific areas of chronic disease management and wider Practice Nursing.
