Job summary

Home First Hub is an integrated discharge team working with our Acute hospitals across Norfolk to support patients through their discharge home, community inpatient unit or to residential placement.

It is a multidisciplinary team of clinical and social professionals dedicated to patient recovery and wellbeing. EEC is seeking an experienced and enthusiastic Occupational Therapist to join our team in providing reablement assessment to a specific cohort of patients across a 5 day service Monday to Friday.

This role will focus on patients who are not able to immediately return home or enter a community in patient unit but require a period of recovery, reablement, and assessment to understand their long term needs and goals. It will involve linking with our team at Norfolk & Norwich University Hospital and visiting patients in their short term placements in residential homes across Norfolk. The specific cohort of patients will be those predominantly with cognitive impairment

Main duties of the job

To contribute to the delivery of high standards of health care to patients within care homes by assessing, planning, implementing and evaluating packages of care.

Be responsible for delivery of care in a reablement and recovery setting.

To support an environment where all team members are responsible for a reablement ethos using a positive risk-taking approach for patients, their carers and families.

To support patients being as independent as possible within their functional limits.

To work as part of a multidisciplinary team within a quality framework to ensure that the highest standards of care are met.

To support and deliver innovation within discharge practice and pathways.

To work with system partners including the voluntary sector to ensure that patients experience is a seamless transfer of care back home or to a long term placement.

Be responsible for the delivery and co-ordination of care to a named group of patients.

Work proactively within the team to achieve unit and trust goals and objectives.

Work with and promote recognised policies, protocols, and guidelines.

Work with HFH team to identify opportunities for learning and improvement specific to this patient cohort.

Job description

  • To undertaketherapy assessments and treatment plans for patients with long term conditions, palliative care and rehabilitation needs to achieve quality of life and independence where possible.
  • To work within the integrated team to facilitate supported discharge
  • To work within the integrated team to prevent unnecessary admission to hospital.
  • To work with all health care professionals, and statutory/non-statutory agencies to provide a seamless, integrated service to our service users.
  • To support learning within the role and patient cohort with a view to understanding and evidencing the longer terms needs for this cohort within the wider NHS and social services.
