Job summary

We are looking for an experienced and compassionate Practice Nurse to join our well-established rural dispensing GP practice for 16 hours per week, working Tuesdays and Fridays.

This is an excellent opportunity for an experienced General Practice Nurse to become part of our close-knit team, where continuity of care, teamwork and building strong relationships with our patients are at the heart of what we do.

The successful candidate will be confident working independently within a General Practice setting and will have previous experience delivering a broad range of Practice Nursing services. The role will include treatment room nursing, Long-Term Condition management, health promotion, preventative care, vaccinations and screening, alongside contributing to QOF and Enhanced Services.

You will work closely with our GPs, HCA and wider practice team to provide safe, effective and evidence-based care to our patients.

We are looking for someone who is caring, motivated and professional, with the clinical experience and confidence to manage a varied Practice Nurse workload.

Previous experience working as a Practice Nurse within General Practice is essential. This role is not suitable for newly qualified nurses or applicants without previous General Practice experience.

Main duties of the job

As our Practice Nurse, you will provide a high standard of clinical care to patients across treatment room and Long-Term Condition services. You will work confidently and independently within your scope of practice, using your previous General Practice experience to deliver safe, effective and patient-centred care.

Your main duties will include:

• Delivering nurse-led clinics for patients with Long-Term Conditions (LTCs), including annual reviews and supporting QOF delivery.
• Providing treatment room services, including wound care, dressings, venepuncture, ECGs, blood pressure monitoring, injections and specimen collection.
• Administering childhood, adult and seasonal vaccinations and immunisations, and undertaking cervical screening where qualified.
• Promoting healthy lifestyles through patient education, NHS Health Checks and preventative care.
• Maintaining accurate clinical records using SystmOne, including appropriate clinical coding.
• Supporting infection prevention and control, maintaining clinical equipment and treatment room stock, and contributing to clinical audit and service improvement.
• Recognising and escalating clinical concerns appropriately and maintaining high standards of patient safety and clinical governance.

Job description

As an experienced Practice Nurse at Hibaldstow Medical Practice, you will play a key role in delivering high-quality, patient-centred nursing care within our friendly rural dispensing GP practice. Working as part of a supportive multidisciplinary team, you will provide a wide range of treatment room services and contribute to the management of patients with Long-Term Conditions (LTCs).

Previous experience working as a Practice Nurse within General Practice is essential. The successful candidate will be expected to work confidently and independently within their scope of practice.

Key Responsibilities

• Deliver safe, effective and evidence-based nursing care in accordance with NMC standards, practice policies and national guidance.
• Assess, plan, implement and evaluate patient care within your scope of practice.
• Deliver nurse-led Long-Term Condition (LTC) clinics, including diabetes, asthma, COPD, hypertension and cardiovascular disease, according to your competencies.
• Undertake structured annual reviews and support the achievement of Quality and Outcomes Framework (QOF) and Enhanced Service targets.
• Carry out NHS Health Checks and provide health promotion and lifestyle advice.
• Undertake a range of treatment room duties, including:
• Wound care and dressings
• Venepuncture
• ECGs
• Blood pressure monitoring
• Injections and medication administration within competency
• Specimen collection
• Removal of sutures and clips
• Deliver childhood, adult and seasonal immunisations and vaccinations, including influenza and COVID-19 programmes.
• Undertake cervical screening where appropriately trained and qualified.
• Promote self-management and preventative healthcare through patient education.
• Recognise deteriorating patients and refer or escalate appropriately.
• Maintain accurate and comprehensive clinical records using SystmOne, ensuring appropriate clinical coding and documentation.
• Ensure treatment rooms remain clean, safe, well-stocked and compliant with infection prevention and control standards.
• Participate in clinical audit, quality improvement and service development.
• Work collaboratively with GPs, the Nursing Associate, Healthcare Assistants and the wider multidisciplinary team to deliver safe, effective and compassionate patient care.
• Maintain professional competence and undertake relevant continuing professional development (CPD).
• Contribute to clinical governance and maintain high standards of patient safety and confidentiality.

Our Team Hibaldstow Medical Practice is a small, friendly rural dispensing practice serving approximately 3,500 patients across a wide geographical area of around 120 square miles. Our close-knit team includes two GPs, two practice nurses, a nurse associate, a healthcare assistant, eight dispensers, four receptionists, two administrators and a practice manager. We work collaboratively to provide high-quality, patient-focused care to our local community.
