Job summary

Are you an experienced nurse looking to develop your specialist career in Inflammatory Bowel Disease (IBD)? We are looking for an enthusiastic and motivated Clinical Specialist Nurse to join our Gastroenterology team at Wexham Park Hospital.

This is an exciting opportunity to play a key role in the delivery and development of high-quality, patient-centred care for people living with IBD. You will work as an autonomous specialist practitioner, providing expert assessment, advice, education and support throughout the patient journey, while working closely with our multidisciplinary teams.

You will be a visible and accessible specialist resource for patients, families and healthcare professionals, supporting shared decision-making, self-management and effective coordination of care. You will work collaboratively across Gastroenterology and the wider Trust to ensure patients receive timely, safe and effective treatment and access to appropriate services.

The role also provides opportunities to contribute to service development, quality improvement, clinical audit, education and the implementation of evidence-based practice and national IBD standards.

Main duties of the job

Provide specialist assessment, treatment, education, and support for patients with Inflammatory Bowel Disease (IBD). Manage nurse-led telephone and email advice services, ensuring timely access to specialist care and support. Coordinate outpatient care, patient follow-up, and ongoing monitoring of clinical symptoms, investigations, and treatment response. Monitor patients receiving biologic and advanced therapies, ensuring safe, effective, and evidence-based treatment. Identify and manage disease flare-ups promptly to reduce complications, emergency department attendances, and hospital admissions. Promote patient self-management, education, and improved long-term health outcomes. Support discharge planning, care coordination, and continuity of care across primary and secondary care services. Work collaboratively with consultants, pharmacists, dietitians, primary care teams, and the wider multidisciplinary team. Ensure compliance with NICE guidance, national IBD standards, and evidence-based practice. Lead medicines optimisation initiatives and contribute to service development, quality improvement, audit, research, and staff education. Develop and implement clinical guidelines, pathways, and best practice. Support increasing service demand through proactive disease management, admission avoidance, and improved patient flow. Contribute to improved patient experience, population health outcomes, cost-effective care, and reduced healthcare utilisation.
