Job summary
<p>Are you highly organised, a confident communicator and someone who enjoys helping people?</p><p>We have a full-time opportunity for a Care Coordinator to join our Care Co-ordination Centre at Gravesham Community Hospital.</p><p>Youll be an important first point of contact for service users, GPs and Integrated Health and Social Care teams across North Kent, receiving and processing referrals and making sure information reaches the right services within the required timeframes.</p><p>Working in a busy environment, youll communicate with a wide range of people throughout your day, answering queries, accurately recording information and supporting the smooth running of our community services.</p><p>This is an ideal opportunity for someone with strong administrative and customer service experience who enjoys working as part of a team and wants their work to have a positive impact on patients and healthcare colleagues.</p><p>The Care Co-ordination Centre operates 7 days a week between 8am and 8pm.
