Main duties of the job

The role includes delivering nurse-led clinics, administering medications and treatments, responding to medical emergencies, undertaking triage assessments, wound care and physical health assessments, and making referrals to specialist services where appropriate.

You will manage a varied caseload and work both independently and as part of the wider multidisciplinary team to ensure patients receive safe, effective and person-centred care.

The role requires sound clinical knowledge, good decision-making skills, excellent communication and the ability to adapt to a busy and varied healthcare environment.

You will work closely with colleagues across primary care, mental health, substance misuse services and the wider prison establishment, helping to ensure continuity of care and positive outcomes for patients.

Job description

The post holder will provide a comprehensive physical healthcare service as part of the multidisciplinary team, delivering safe, effective and high-quality care to patients within a secure healthcare environment.

Working in partnership with colleagues across primary care, mental health, substance misuse services and the wider prison establishment, the post holder will contribute to an integrated approach to healthcare and support the management of a wide range of physical health needs.

The role requires the ability to work both independently and collaboratively, using sound clinical judgement to assess patients, prioritise care and respond appropriately to changing or urgent healthcare needs.

Key responsibilities include:

• Undertaking comprehensive health assessments, clinical reviews and ongoing monitoring of patients.
• Delivering nurse-led clinics and evidence-based interventions within scope of practice.
• Undertaking triage and responding to urgent and unplanned healthcare needs.
• Responding to medical emergencies across the establishment as part of the wider healthcare emergency response, working alongside and with the support of healthcare colleagues and established local procedures.
• Administering medications, treatments and vaccinations safely and in accordance with relevant policies and procedures.
• Providing wound assessment, treatment and ongoing wound management.
• Undertaking physical examinations, observations, diagnostic procedures and investigations within scope of practice.
• Assessing, planning, implementing and evaluating individualised programmes of care.
• Identifying changes or deterioration in a patients condition and escalating or referring appropriately.
• Making referrals to specialist, secondary care and other healthcare services where required.
• Supporting the management of long-term conditions and promoting preventative healthcare.
• Providing health promotion, education and advice to support patients to make informed decisions about their health.
• Working collaboratively with internal and external healthcare professionals to support continuity and coordination of care.
• Maintaining clear, accurate and contemporaneous clinical records in accordance with professional and organisational standards.
• Working in accordance with safeguarding, infection prevention, medicines management and clinical governance requirements.
• Contributing to clinical audits, quality improvement, service development and learning from incidents.
• Maintaining professional competence and participating in training, supervision and continuing professional development.

We recognise that secure healthcare may be a new environment for many applicants. You will be supported to develop your understanding of prison healthcare through induction, training and working alongside experienced colleagues. There is an expectation that nurses develop the confidence and competence to manage the full range of responsibilities within the role, but this development is supported as part of the wider healthcare team.

The post holder will be expected to work flexibly as part of the wider healthcare team and contribute to the delivery of a responsive, patient-centred service across HMP Humber.
