Job summary

To act as an autonomous practitioner who is able to work independently and in conjunction with other members of the clinical team. The post holder will be an experienced caring nurse with a specialty in diabetes who can demonstrate developed clinical skills and be able to demonstrate critical thinking whilst working within their professional boundaries. They will work the Surgery clinical team, consisting of GPs, ANPs, Practice Nurses and Health Care Assistants to deliver excellent primary care services within the practice.

This role will demonstrate safe, competent clinical decision-making and deliver expert care, including assessment, diagnostic skills, treatment and will communicate at all levels across boundaries.

The post holder will work collaboratively with the multidisciplinary team to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required

Main duties of the job

The position is Part-Time 24-27hrs per week. The role is structured with an organised work flow, with set appointment numbers and the Postholder should expect a mix of same-day and pre-booked diabetic review encounters.

Main Purpose of Post: Triage and consultations. The role involves telephone triage, chronic disease management and medication monitoring. The focus of care evolves around our diabetic patients.

Location: Postholder will be required to work at The Thorpewood Surgery or

Dussindale Branch Surgery as required.

Main duties and responsibilities of the job:

A point of contact within the practice for diabetic patients who require review and management of their chronic disease, that require either telephone or Face to Face consultation.

Assessment, monitoring, diagnosis, treatment, prescribing, referral as appropriate and deliver key elements of Surgery initiatives.

Instigate invasive and non-invasive diagnostic tests or investigations, interpretation of findings and reports.

Directly admits patients to secondary care outlets in medical need.

Refer patients to other medical specialities for assessment where indicated.

To take responsibility for making autonomous decisions in relation to patient care.

Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly.

Assess, diagnose, plan, implement and evaluate interventions/treatments for patients with complex needs.

To collect data and participate in regular Practice audit and research projects

Work effectively with individuals in other agencies to meet patients and carers needs

Recognise and work within own competence and professional code of conduct as regulated by the NMC

Produce accurate, contemporaneous and complete records of patient consultation, consistent with legislation, policies and procedures on the Surgery clinical system

Deliver care according to local and national guidelines and evidence-based care

People Management:

Provide expert Diabetes and nursing advice to others working within the team.

To maintain own 3 yearly registration with the NMC

To maintain own continuous professional development and ensure PDP kept up-to-date in line with the requirements of the Practice

Strategic Management

Work with the clinical and management team to develop new services for patients

Contribute to practice incentive schemes, disease management registers and Surgery targets set locally and nationally for patient care.

Special Requirements of the Job:

An understanding, acceptance and adherence to the need for strict confidentiality.

Competence in the use of IT systems, i.e. coding work as completed, with the ability to learn new software.

General

Non medical prescribing qualification desirable but not essential.

Care Quality Commission

To ensure compliance with Surgery policies, reporting any deficiencies, potential hazards or dangerous occurrences to the Patient Services Manager or Practice Manager.

Participate and support Health & Safety statutory requirements and initiatives. Maintain records of incidents and undertake risk assessments.

Compliance with the Surgery confidentiality and Information Governance Policies.

Take responsibility for your own personal Development, including completing the annual appraisal and undertaking subsequent activity identified and agreed with the line manager.
